Pre-Enrollment Now Open

People who experienced the Red Hill water crisis are encouraged to complete the Pre-enrollment questionnaire as the first step toward enrolling in the Registry. Interested individuals may also sign up for updates on the Registry’s progress.

The brief 5–10 minute questionnaire helps connect you with the Registry and shape a community-driven approach that reflects the real needs and experiences of the Red Hill community. Participants must be 18 or older to complete the form, but may also provide information for children under the age of 18 or family members for whom they are a legally authorized representative.
